How register Toyota Vellfire

I have recently imported Toyota vellfire 2014. I am struggling to fill V55/5 form to register it. If anyone have done it before I appreciate if you can help me with filling the form as there are some questions which I can’t find an answer as I did not get a Certificate of Conformity with Vellfire.
